If you would like to request a copy of a death certificate, please complete the form below:

Once completed, you can bring the form in to the office at 35 Woodfin St, mail it in to 35 Woodfin St, email it to register@buncombecounty.org, or fax it to 828-250-4333.

Certified copy of a Death Certificate

  • $10.00/certificate
  • To obtain a certified copy, you must be related to the person whose document you are requesting, unless you are an Authorized Agent, or an Attorney. This includes spouse, sibling, child, parent, grandparent, or grandchild, also included are step relationships.

Uncertified copy of a Death Certificate

  • $0.25/certificate
