We provide case management services including:
- outreach to community families;
- support and case management for eligible pregnant and postpartum women and families with (delete the word “young”) children from birth to 5 years old).
Through these services we strive for reducing infant deaths, improving child health and development, increasing access to community services and resources, and improving quality of life.
Social Work Services include:
- Child Service Coordination (CSC) can:
- Help families discover and discuss their strengths and concerns.
- Provide support by locating programs, services and resources that meet the family needs.
- Serve as a link between families and their child’s doctor.
- Discuss ways families can strengthen parent-child relationships.
- Introduce families to parent support programs and offer encouragement and supportive counseling.
- Health Check Coordination (HC) can:
- Track and provide educational services to Medicaid eligible children from birth to 21 years.
- Update and reschedule health care needs.
- Promote well child care and immunizations in the community.
- Maternity Care Coordination (MCC) will provide:
- Case management assistance for pregnant and postpartum women
- Referrals to community resources
- Assistance with Medicaid, WIC, and other applications
- Supportive counseling
- Help in making and keeping prenatal appointments.
Hours of Operation:
Monday through Friday:
8:00am - 5:00pm
How are services paid for?
Social Work Services accepts Medicaid insurance for case management services. There is no charge to eligible clients who do not have Medicaid.
